According to Books Online, you can only add or drop a column and it must be done in the replication configuration screen in Enterprise Manger. See the Replication topic in the index. Underneath it, you will find schema changes topic.

Have a look at sp_repldropcolunm and sp_repladdcolumn

DTS can be used in a similar way.
