I could use a hand. I'm wrapping up a project and need to document my stored procedures, tables and other info such as indexes, constraints, triggers, etc. I'm not much of a documenter. Does anyone have any decent examples of format, or a website where I might find some?

This is a WAG, but maybe Visio does this?
