thus a table like this :

Date person changed

6/1/01 Alex Form 3
5/1/01 Alex Form 5
4/1/01 Sid Form 4

2/1/01 Jeff Form 8
1/1/01 Sid Form 1

Becomes :

Date person changed

6/1/01 Alex Form 3
4/1/01 Sid Form 4
2/1/01 Jeff Form 8
